Job Description:
A company is looking for an RN Case Management Analyst to
promote health outcomes for members through case management
processes.
Key Responsibilities
Assess, plan, implement, coordinate, monitor, and evaluate health
services for a defined population
Establish collaborative relationships with clients, families, and
healthcare providers to optimize health outcomes
Develop and maintain patient-centric case management plans while
adhering to quality assurance standards
Required Qualifications
Active unrestricted Registered Nurse (RN) license in the United
States; Compact RN license required
Two years of full-time equivalent direct clinical care
experience
Previous case management experience, preferably with the
Medicare/Medicaid population
Strong personal computer skills, including MS Word, Excel, and
Outlook
Bilingual (Spanish) and CCM certification preferred
