LanceSoft is seeking a local contract nurse RN Care Manager for a local contract nursing job in brigham city, Utah. Job Description & Requirements - Specialty: Care Manager - Discipline: RN - Duration: 52 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract Position Summary The Registered Nurse (RN) Case Manager coordinates and provides skilled nursing care to home health and hospice patients. This role focuses on patient education, compassionate care, and close coordination with the interdisciplinary team to ensure the best possible outcomes for patients and families. Key Responsibilities Patient Care • Develop and implement individualized plans of care with patients, caregivers, and care team members. • Deliver skilled nursing care according to physician orders. • Educate and support patients and families regarding treatment plans and disease management. • Accurately document visits, updates, and changes in patient condition. • Assist with discharge planning and ensure continuity of care. Communication & Coordination • Maintain ongoing communication with physicians, patients, and interdisciplinary team members. • Participate in case conferences and agency quality initiatives. • Coordinate necessary medical equipment and community resources for patients. Leadership & Support • Supervise and evaluate Home Health Aides and LPNs/LVNs. • Participate in on-call rotation and staff training programs. • Contribute to continuous quality improvement efforts across the agency. Additional Position Details Travel Area: • Service areas include Box Elder County (from Smith & Edwards in Farr West to the Idaho border) and Cache County (to the Idaho border). • Garden City is not currently part of the service area. Patient Load & Visit Structure: • The agency uses a point-based system that varies by visit type: o Regular visit = 1 point o Recertification = 2 points o Admission = 3 points • An average workload is 25–30 points per week. • Visits should not be less than 15 minutes. Travel Requirements: • Travel is required. Patients are grouped by service area when possible, but due to branch size, additional driving (“windshield time”) may occur. • Average daily travel area covers Box Elder and Cache Counties. • Mileage reimbursement provided at $0.41 per mile. Typical Schedule: • Regular hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. • Includes rotating on-call and holiday coverage.
