We are known for our responsive leadership, collaborative clinical team, and commitment to excellent outcomes. We are currently seeking a Physical Therapist (PT) to join our growing home health team. This role is ideal for a motivated therapist who values autonomy, flexibility, and meaningful one-on-one patient care. Position Overview The Physical Therapist provides skilled physical therapy services to home health patients, focusing on improving mobility, strength, safety, and independence. The PT works collaboratively with physicians, nurses, and the interdisciplinary team to develop and implement individualized plans of care. Responsibilities Perform initial and ongoing physical therapy evaluations in the home setting Develop and implement individualized treatment plans based on patient goals and clinical needs Provide therapeutic exercises, gait training, balance training, and functional mobility interventions Educate patients and caregivers on safety, fall prevention, and home exercise programs Communicate and coordinate care with physicians and interdisciplinary team members Accurately complete documentation in compliance with state and federal regulations Participate in care conferences and interdisciplinary meetings as assigned Qualifications Current Utah Physical Therapist license (required) Home health experience preferred but not required Strong clinical assessment and critical-thinking skills Excellent communication and documentation abilities Ability to work independently and manage a flexible schedule Valid drivers license and reliable transportation What We Offer Competitive pay (per visit or salary options, depending on status) Flexible scheduling (full-time, part-time, or PRN opportunities) Reasonable caseloads and strong clinical support Supportive, accessible leadership team Locally owned organization focused on quality, not volume Opportunity to make a meaningful impact in patients lives How to Apply If you are passionate about delivering high-quality therapy in a home-based setting, we would love to hear from you.
