GENERAL SUMMARY:

Under the direct supervision of the Laboratory Manager, the Lab Assistant collects blood and other laboratory specimens, log specimens, prepares the samples for testing and analyzes the specimens that require CLIA waived and moderate complexity testing as requested following adequate documented training.

GENERAL RESPONSIBILITIES:

1. Provide professional and courteous service to all "customers" of Ogden Clinic. Customers include all patients, affiliates, administration, employees, co-workers, insurance company representatives, and all other vendor representatives of Ogden Clinic.

2. Meet behavioral expectations and support the dignity of all persons. Also responsible for the stewardship of resources.

3. Comply with regulatory requirements and guidelines that maintain safety for oneself, patients, visitors, and co-workers.

4. Study, understand, and adhere to all Ogden Clinic Policies and Procedures.

ESSENTIAL JOB FUNCTIONS:

1. Complies with privacy and security standards promulgated in the Health Insurance Portability and Accountability Act (HIPAA) and the Health Information Technology for Economic and Clinical Health Act (HITECH).

2. Transmits laboratory orders generated in the EMR (eCW) by providers or the providers staff to the LIS (Orchard) accurately and efficiently

3. Retries stored orders in the LIS (Orchard) that have been generated in the EMR (eClinical Works) to produce specimen labels accurately and efficiently.

4. Responsible for notification of inappropriately labeled specimens, to the lab manager and provider and/or reference lab if appropriate.

5. Understands and has knowledge of proper phlebotomy procedures to obtain samples from patients of all age groups.

6. Labels specimens with patient's full name and a second identifier immediately after collection. This requirement assures positive identification and optimum integrity of patient specimens from the time of collection until testing is completed and results reported.

7. Labels specimens and requisitions appropriately.

a. Properly handles ambient, refrigerated, and frozen specimens

b. Able to recognize all specimen types

c. Compares the name and other identifiers on the LIS label, verify all names/identifiers match

d. Properly labels each specimen(s)

e. Properly handles specimens requiring a Reroute or to be share

f. Writes prompt information on appropriate specimen label(s)

g. Labels appropriate aliquot tube(s) for splitting

8. Able to process specimens at all three temperatures and follow the appropriate procedures for all manual and electronically transmitted orders

9. Aliquots specimens according to procedure/test requisition using the proper safety equipment.

10. Understands appropriate specimen collection, handling and transport procedures. Prepares specimens for analysis and determines acceptability of samples within guidelines.

11. Remains informed of all procedure changes pertaining to specimen processing.

12. Observes principles of data security and patient confidentiality. Maintains ethical standards in the performance of testing and in interactions with patients, co-workers and other health care professionals.

13. Performs and documents quality control and assists with equipment maintenance within predetermined parameters. Does not calibrate instrumentation unless assisted by instrumentation specialist or lab manager.

14. Transmits results via an interface or automatically by a testing instrument.

15. Develops proficiency with laboratory information system (Orchard).

16. Answers telephone calls in a timely and courteous manner. May provide verified test results available in the LIS, as requested, to authenticated individuals.

17. Refers problems to appropriate technical personnel.

18. Maintains work area. Cleans bench tops and equipment after each spill and daily at the end of the work shift. Documents cleaning and maintenance as required.

19. Stocks and maintains supplies. Notes receipt date on supplies and rotates supplies to ensure that the oldest stock is used first. Discards expired reagents in a manner consistent with applicable safety policies or procedures. Notifies the appropriate supervisor or designated individual, as needed.

20. Assists others in areas of defined competency when assigned workload is complete.

21. Reports any product or result anomalies to the lab manager.

22. Generates and maintains accurate records and documentation.

23. Performs any required and/or requested calibration of selected laboratory equipment and instrumentation to ensure accuracy.

24. Performs and generates equipment and instrument quality control documentation and ensures that equipment and instrumentation is properly functioning and meets acceptable standards.

25. Performs waived (ESR's, Urine chemistry dips, Whole Blood mono's, Rapid Strep screens, RSV's, Urine hCG's, INR's, and Fecal Occult Blood tests) moderate complex (CBC with automated differential, Serum hCG, and Influenza A and B) analytic processes without direct supervision. Uses routine and specialized automated and non-automated laboratory procedures and/or techniques for which he/she is qualified, trained, and demonstrates competency according to established lab standard operating procedures (SOP's).

26. Operates laboratory instruments and ensures proper functioning of laboratory equipment. Troubleshoots basic instrument malfunction(s) and performs preventive and corrective maintenance on all laboratory equipment. Uses instrument hotline(s) if available to obtain help in resolving instrument problems. Recognizes when professional service is required and requests service if instrument is covered under a service contract. Otherwise, reports service needs to Lab Manager for resolution. Maintains records and documentation of maintenance.

27. Calibrates laboratory instruments to ensure accuracy of test results. Performs quality control procedures as specified in lab section SOP's and maintains QC records and documentation necessary to meet the standards of accrediting agencies.

28. Processes patient testing data and enters data in the laboratory computer system, along with appropriate explanatory or interpretive information.

29. Monitors quality control and assists in data collection for QA indicators, as assigned.

30. Performs internal and external proficiency testing (PT). Handles PT samples in the same manner as patient samples.

31. Meets output measures as defined by individual group policy. Assists with workload in any section where competency has been verified and is current.

32. Performs new procedures following appropriate training and as directed by the Lab Manager.

33. Must be able to demonstrate competency in trained areas and functions.

34. Assists others in the other areas of demonstrated competency when assigned tasks have been completed.

35. Stocks and maintains reagents, supplies and may be requested to assist in inventory accountability.

36. Understands and follows microbiology procedures for setting up cultures

37. Process specimens for referral to outside laboratories.

38. Perform waived and moderate complexity testing upon proper training

39. Recognize testing inconsistencies and takes appropriate corrective action after consulting with an MT/MLS or MLT/MLS

40. Review reports for clerical accuracy

41. Report all critical test results to the physician or MA immediately according to established policies.

42. Perform and run QC according to procedure

43. Analyze QC data before reporting patient results and takes appropriate corrective action when results are outside acceptable limits.

44. Seek consultation from MT/MLS or MLT/MLS and/or lab supervisor to resolve QC problems and document corrective action taken.

45. Log all out of range QC results

46. Observe and follow department preventive maintenance schedules and document all instrument checks, preventive maintenance, problems and corrective action taken.

47. Perform office or clerical duties when necessary(i.e. filing, billing, answering phone calls, data entry)

48. Attend all department and clinic staff meetings and in-services.

49. Attend and participate in all laboratory continuing education and assigned Proficiency Testing events.

50. Read all laboratory and clinical policies and procedures annually

51. Keep work area neat and clean. Restock daily supplies

52. Do not eat or drink in the laboratory

53. Maintain equilibrium in stressful situations

OTHER DUTIES:

1. Reads all policies and procedures annually and signs the attestation statement.

2. Complies with safety regulations as defined by Ogden Clinic policy.

3. Attends laboratory meetings and in-services as required.

4. Remains informed of all relevant process and procedure changes.

5. Adheres to all Ogden Clinic and Laboratory guidelines, policies, and procedures.

6. Checks with Supervisor or Lead before making judgment decisions.

7. Performs cleaning duties as assigned.

8. Adheres to all safety policies and regulations including, but not limited to, proper dress code and using proper protective clothing and other equipment.

9. Management has the discretion to change any duties as needed.

QUALIFICATIONS:

1. EDUCATION: High School diploma or GED equivalent. Successfully completed a phlebotomy program with certification.

2. LANGUAGE SKILLS: Proficient in reading and interpreting documents, have a good knowledge base of lab tests and specimen requirements.

3. Medical terminology knowledge required.

4. KNOWLEDGE BASE

a. Understands and complies with good laboratory practice, including safety, HIPAA, confidentiality, and regulatory requirements.

b. Recognizes critical patient results and follows the defined process for reporting them.

c. Recognizes unexpected results, errors and problems with patient tests and escalates as required.

d. Understands the theory of laboratory procedures. Evaluates potential technical, instrumental, and/or physiologic causes of unexpected test results.

e. Correlates clinical and laboratory data with pathologic states to determine result validity.

f. Evaluates exceptions to established criteria and special circumstances related to specimen collection and integrity to determine impact on testing.

g. Recognizes appropriate and inappropriate selection of basic and complex lab tests.

h. Observes principles of data security and patient confidentiality. Maintains ethical standards in the performance of testing and in interactions with patients, co-workers and other health care professionals
